BOOST stands for 'Bringing about
Optimal
Outcomes through
Self-care and
Technology, an
app-based learning programme designed to empower patients to make changes and monitor their own health through education and support. It has a range of functions and a
questionnaire for patients to better understand their overall well-being.
Patients may look forward to a personalised self-care report and decide on the health goals they would like to set. They will receive motivational and engaging educational material in a variety of formats, while capturing and monitoring their own glucose levels, blood pressure and weight over the months. There will also be rewards to encourage and incentivise for healthy behaviour changes.
