​A medication list is a record of the medications you are currently taking. It should ideally include medications prescribed by hospitals, polyclinics, or your GP, as well as over-the-counter medications and supplements.

Maintaining a medication list ​ ensures that your healthcare providers are aware of the medications you are taking and any drug allergies you may have. This prevents them from prescribing medications that are not suitable with your current medications.

Bring it to every appointment or pharmacy visit to keep your healthcare providers informed. Remember to update the list whenever there is a change.
