In 2019, NHG published ‘River of Life: NHG’s Perspectives on Population Health’, an encapsulation of our on-going transformation from care provider to care partner.
As the Accountable Care Organisation (ACO) for the Central Region, NHG is moving beyond episodic care and treating illnesses to fostering the holistic health of the population with our River of Life (ROL) framework. This blueprint guides our evolving approach to health – which includes the physical, mental and social dimensions – and is founded on the principle that everyone wants to live well from cradle to grave. ROL is framed by our Five Segments of Care – Living Well, Living with Illness, Crisis and Complex Care, Living with Frailty, and Leaving Well – and is implemented in our three geographic zones: Central Zone, Yishun Zone, and Woodlands Zone.
This publication provides insights into the drivers of change (described as the ‘Three Waves’ of Frailty, Chronic Disease, and Poor Lifestyle Choices), our strategy for population health and initiatives, as well as key enablers such as funding models, technology and innovation, human resources and organisational processes to increase productivity and provide greater value in our healthcare delivery.
