Caring for Central and North Singapore

National Healthcare Group (NHG) cares for the population of 1.5 million residents of Central and North Singapore as the Regional Health Manager (RHM).

In caring for the health of all residents in Central and North, and not just patients, NHG goes beyond running hospitals and polyclinics. Shifting from provider of care to partner in care for residents, NHG collaborates with private general practices, community health and social care providers, to keep residents well and healthy, as well as enable residents to care for their own wellbeing.

With more than 20,000 healthcare professionals in NHG, we are committed towards building healthier and resilient communities, and Adding Years of Healthy Life to the people we serve.

How We Care

In caring for our residents, NHG considers the needs of our residents along their River Of Life, from living well to leaving well.

Our model of care sees residents at the centre, active and enabled to take ownership of their wellbeing and care. The resident is supported by strong relationship with a family doctor for preventive and primary care. They in turn find wider support within place-based communities of care that consist of networks of community providers for further health and social needs, and ageing well in place, as well as caring communities of friends, neighbours and volunteers. When unwell, residents are supported by integrated care that joins up care for their different health and social needs across different settings of hospital, community and home.

As regional health manager, NHG supports this care through collaborating with and enabling family doctors, community providers, caring communities, and other healthcare providers. It also directly supports residents as NHG Cares, a suite of services that includes NHG Cares App, NHG Cares Hotline, and NHG Cares Membership.
