Community Care advances care and well-being of residents by facilitating accessible,​ comprehensive preventive healthcare services, developing place-based communities of care (CoCs) in neighbourhoods, schools and workplaces, and enable and facilitate shared care and care transition efforts anchoring patients and residents in th​e community.​
