​Population Health Campus

The NHG Population Health Campus (PHC) brings together NHG’s ​population health efforts as the ‘fourth campus’ to better serve our 1.5 million assigned residents.

As a ‘campus’ that covers the whole of Central and North Singapore, PHC partners our hospital campuses, specialty centres, GPs and community providers, to join up care, integrate health and social care, take a preventive health strategy and support ageing in place.

​​Population Health

Population Health drives the transformation of our pre​​ventive care and aged care systems through strategic planning and development, with robust data, focused on value. Enhanced​ access to digital, telehealth, diagnostics and pharmacy services and strong collaborations with provider, partners and residents facilitate efficient, joined-up quality care.

​Community Care

Community Care advances care and well-being of residents by facilitating accessible,​ comprehensive preventive healthcare services, developing place-based communities of care (CoCs) in neighbourhoods, schools and workplaces, and enable and facilitate shared care and care transition efforts anchoring patients and residents in th​e community.
Primary Care

Primary Care, comprising polyclinics and general practitioners in Central and North, serves as first point of contact for healthcare. With a person-centred approach and focus on prevention, early detection and management of residents’ and patients’ health, Primary Care continually innovates to transform primary care practice towards integrated and right-sited care.​​​​​​​​​​​​​​​​​​​​​​​
