The NHG Central-North Primary Care Network (CN-PCN) is a network of GPs
that is supported by nurses and care coordinators. It brings care closer to the
community by providing ancillary and support services to patients with chronic
conditions.
As of 31 July 2019, the NHG CN-PCN has 38 GPs in its network. These GPs
in the network are able to provide patients with access to services such as
nurse counselling, and diabetic eye and foot screenings.
Beyond offering shared support services to GPs, the CN-PCN ensures that
these GPs remain connected to the larger healthcare landscape, and also supports
them in medical education and training. Platforms are also available for them
to hear from subject matter experts and offer networking opportunities with
leaders from partnering Regional Health Systems.
