We collaborate with General Practitioners and Polyclinics to facilitate patient access to care for acute ailments and the management of chronic conditions. Our aim is that each patient is assigned a dedicated Primary Care physician, to ensure seamless patient care integration during transition between community and hospital settings. By ensuring every patient has a designated Primary Care physician, we foster continuity of care and enhance patient outcomes across different healthcare environments. As part of the national Healthier SG plan (Healthier SG​​​), we collaborate with GPs to assist residents in achieving their health goals and connect them with community resources such as programmes, activities and services. ​​​
