                    ​Overview​​​

We collaborate with General Practitioners and Polyclinics to facilitate patient access to care for acute ailments and the management of chronic conditions. Our aim is that each patient is assigned a dedicated Primary Care physician, to ensure seamless patient care integration during transition between community and hospital settings. By ensuring every patient has a designated Primary Care physician, we foster continuity of care and enhance patient outcomes across different healthcare environments. As part of the national Healthier SG plan (Healthier SG​​), we collaborate with GPs to assist residents in achieving their health goals and connect them with community resources such as programmes, activities and services. ​​​

ONE Relationship ​Team​

At National Healthcare Group (NHG), our Relationship Team is dedicated to support you in preventive health efforts for your residents in the Central North of Singapore. Under Healthier SG and NHG population health initiatives, we work with you in caring for your patients and keeping them well in the community.
